The oxygen tube end lay on the floor and was not connected to the machine. The albuterol mouthpiece was in a partially closed ziplock bag on the floor. Three light bulbs in two bathrooms were burnt out. Two wheels on the bedside table were broken, making it inoperable. The toilet had a water ring indicating it had not been cleaned. The floor was dirty and sticky, clearly not properly cleaned. The reclining chair was stained, dirty and disgusting, prompting a request for a sheet. The wall-mounted hand sanitizer was empty. The staff was unfriendly.
